First trimester maternal serum levels of human placental lactogen and beta-subunit are independent predictors of birth weight.
To evaluate the relationships of maternal serum levels of placental lactogen (PL) and human placental beta-subunit (HPL-B) to birth weight. Data from 1,182 women who delivered a singleton infant were used to develop and validate a mathematical model to predict birth weight based on maternal serum levels of PL and HPL-B. Receiver operating characteristic (ROC) curves were constructed for predicting birth weight. Two independent birth weight populations were evaluated. In the first population, 862 women were used to develop the model, and validation was done in 30 women. In the second population, 500 women were used to develop the model and 200 women were used for validation. The following variables were independently associated with birth weight: maternal age, maternal race, maternal weight, parity, gestational age, second trimester maternal serum levels of alpha-fetoprotein, unconjugated estriol, human chorionic gonadotropin, hemoglobin, total cholesterol, albumin, and PL and HPL-B levels. We developed a model that included maternal age, maternal weight, parity, gestational age, second trimester maternal serum levels of alpha-fetoprotein, unconjugated estriol, human chorionic gonadotropin, hemoglobin, total cholesterol, albumin, and PL and HPL-B levels that predicts birth weight with an accuracy of 80.7% (95% confidence interval, 76.5%-84.6%) in the first population and of 81.4% (95% confidence interval, 73.4%-88.8 be359ba680
